Hand-off is a silent loss zone
Many plants know the visible cost of a breakdown, but undervalue the accumulated cost of poor shift changes: repeated incidents, tests already run but never recorded, pending decisions duplicated, and micro-stoppages that look isolated.

Errors appear when transfer depends on oral memory, scattered chats, or personal notes. In that setting, information does not disappear completely, but it does lose consistency and recoverability.
